The public is hereby informed that the PCSO’s Medical Access Program (MAP) in the National Capital Region (NCR) shall continue to be available thru its Charity Assistance Department. We will continue to accept MAP NCR Applications subject to the accomplishment of the queuing form which is available at pcso.gov.ph
The NCR Online MAP Application System is temporarily suspended from April 26 — 28. 2021 to give way to the transition from data servers to Cloud.
The procedure shall be as follows:
- Check the available medical assistance that may be availed of and the documentary requirements as herein below provided
- Go to the PCSO website pcso.gov.ph, click Downloads. click Charity Program and Services and click IMAP Application Form
- After filling-up the IMAP Application Form, scan or take a photo of it for submission
- Ready the cornplete documents from the checklist of the medical assistance to be requested to be attached to the accomplished Application Form. ONLY APPLICATIONS WITH COMPLETE DOCUMENTS SHALL BE PROCESSED ON A FIRST COME-FIRST SERVE BASIS
- Fill up the Queuing Form under the E-Services and click Subrnit
- Receive acknowledgement, queuing number and instructions from the designated official ernail of PCSO
- Submit electronically the accomplished IMAP Application Form and the documentary requirements which must be complete. clear, and legible
- Receive Claim Slip and Guarantee Letter (GL) electronically within the day
- Print Claim Slip and GL
- Submit printed Clairn Slip and GL to the hospital. health facility or rnedicine retailer with the complete documents required in the checklist within 30 days from the date of the issuance of the GL
CHECKLIST OF DOCUMENTARY REQUIREMENTS:
0 Confinement
o Original/certified xerox copy of medical abstract with license number and signature of the doctor
- Official Statement of Accounts with signature of the Billing Officer less Philhealth and other deductions o valid ID of the patient (photocopy)
Erythropoietin (dialysis in)ection)
o Original/certified xerox copy of medical abstract with license number and signature of the doctor
- Original prescription of epo injection with license number and signature of the doctor
o Original endorsement with official quotation (applicable for Dialysis Center and hospital)
o Valid ID of the patient (photocopy)
0 Chemodrugs
- Original/certified xerox copy of medical abstract with license number and signature of the doctor
o Original prescription with license number and signature of the doctor
o Treatrnent protocol with license number and signature of the doctor
o Valid ID of the patient (photocopy)
(I Hemophilia and Post Kidney transplant medicines
o Original/certified xerox copy of medical abstract with license number and signature of the doctor
o Original prescription with license number and signature of the doctor
o Valid ID of the patient (photocopy)
Hemodialysis
o Original/certified xerox copy of medical abstract with license number and signature of the doctor
o Original endorsement with official quotation frorn the Dialysis Center and hospital
o Certification of Philhealth benefits availment from the Dialysis Center and hospital or Certification of Philhealth exhaustion
o Valid ID of the patient (photocopy)
Requests for medical assistance shall be available from Monday to Friday.
Submission of applications shall be open from 10:00 AM for OUTPATIENT requests including medicines. chemotherapy, erythropoietin injection and hemodialysis. and from 11.00 AM for CONFINEMENT requests UNTIL THE MAXIMUM TOTAL OF 180 CASES IS REACHED BASED ON THE ALLOWABLE DAILY BUDGET FOR MAP NCR.
Cases submitted beyond the maximum number per day should reapply the next day by accomplishing the Queuing Form.
