BACOLOD City – The city government is urging Bacolodnons included in the priority list for coronavirus disease 2019 (COVID-19) vaccination to register online.
The COVID-19 electronic immunization registry (CEIR) is part of the city’s vaccination roadmap, according to Mayor Evelio Leonardia.

The following information are collected:
* complete name
* contact number
* complete address
* sex
* date of birth
* civil status
* employment status
* direct interaction with COVID Patient (Yes or No)
* profession
* name of employer
* address of employer
* contact number of employer
* pregnancy status (pregnant or not pregnant)
* allergies
* other health conditions
* COVID-19 status (if applicable)
* electronic consent
All information will be stored in a database.
Once the vaccine applicant’s information is submitted, the system will evaluate to infer if the applicant is eligible to receive the COVID-19 vaccine.
Eligible individual for vaccination will then be contacted.
Vaccine applicants could also sign up through their respective barangay council offices or employers.
Furthermore,only the Bacolod City Government Emergency Operations Center (EOC), Bacolod City Health Office and the Department of Health will have access to the vaccine applicant’s information that is stored in the system’s secured server.
“Only authorized personnel of CEIR Master List thru Google Forms have access to the information provided by COVID-19 vaccine applicants,” part of the CEIR read.
The city’s population of priority included healthcare workers, senior citizens, indigents, military and uniformed personnel, other frontline workers, and people with special needs.
“Additional vaccines for other residents will come from the supply to be provided by the national government or the city may procure more,” Leonardia said.
Bacolod targets to vaccinate 68 percent of its population.
The city government already made an initial payment of more than P31 million to British pharmaceutical firm AstraZeneca for the 650,000 doses of COVID-19 vaccine it ordered.
The vaccines’ expected arrival is by the third quarter of the year.
The city government has also identified 40 vaccination centers. Each center can have a daily maximum target of 500 vaccinated people, or an estimated 20,000 individuals a day, Leonardia said./PN
